Most of the women attending our maternity services are given their clinical notes (chart) for the duration of their pregnancy. The notes are generally given to each woman when she returns for her second visit to the hospital. The clinical notes are retained by us until the laboratory results are filed or if the woman is attending a high risk clinic. If a women is admitted during her pregnancy the notes my be retained following her discharge. Following the birth the notes are retained by the hospital.
Birth and death notifications are forwarded to the Central Statistics Office, General Registrars Office and Dr. Steevens Hospital.
